Doctors case discussions on 1/12/2016

Dr.pradp Noori:
Dear family,
Excuse us sir for chota and choti about the protocol .intention is that, this helps the new-comers .
When a pt comes in, they give their complaints not the detailing of the case .starting from how to go into details , what to ask ,how to get proper information from PT ( as VD Ganesh sir asked about the lifestyle, Vegas ect) what is the  differential diagnosis, where we should not get confused. This is the training programe intented.
WITH MINIMAL INPUTS WE NEED TO START A SUCCESSFUL TREATING JOURNEY.the information is a discussion subject and all the vaidyas are expected to participate from the basic nidana to chikitsa. I hope all will participate AS A DUTY TOWARDS AYURVEDA. .there are more than 100 startup vaidyas who want to know this but unable to ask .this is an off beat but for the starter's HELPING HAND.
Chota and choti are starters in practice .these charecters representing are, not so confident, unable to ask but who wants to do a pure ayurvedic practice, earn their rice roti and dal , leading a respectable life of an ayurvedic Bhishak.
Expecting a little bit participation from all
Thank you

@Vd Gopikrishna ji. But this is not scientific, as this schedule don't address any per titular disease.

vijayalakshmi:

What are the symptoms? Why she is undergoing hysterectomy? Why hrt has been advised?

DR. P. SRIDHARMURTHY, MYSORE.:

As pt is having DUB since 4 yrs

Payal K:

Kindly share everybody's experience on DUB

What helps...I have used many preparations depending upon situation like pushyanuga choorna, chandraprabha, bolbaddha rasa, nagkeshar choorna, kanchanar guggulu ( if kukkuutnakhi guggulu) is not available, Ashok, lodhra, Shatavari choorna as well

You can give virechana...Also add kaishor guggulu, sariva, manjishtha, vidanga, bakuchi, sookshma Triphala in your previous meds.

If there is lot of itching tell patient to add a piece of camphor on coconut oil and apply the oil on the skin. It helps to relieve itching

Dr.sunita:

Same case I too need info...one young lady aged 28 yrs ...moderately build ... in two years span ...uterus +one ovary and in next year the other ovary too removed bcos of cystic growths.. .non malignant ... now she's gaining weight ... lethargy ... other parameters are normal no h/O DM...HTN...THYROID ...

How to treat ... Ayurveda, line of management for HRT... and Estrogen balance and what all we can do in Hysterectomy  oophorectomy patients ... are we can simply treat in perspect of  menopause and associated issues ??? Please guide and share if any previous treated cases ...thank U

Dr.ravndr:

I always suggest all my veteran friends to present in ayurveda understand when we expect ayurveda treatment. This in turn help both Choti n Cora to understand ayurveda in a better way. Otherwise, treatment suggested is only a quackery.

 For instance,

In the ongoing discussion there are 3 key words viz HYSTERECTOMY, HRT and DUB.

Two queries left here are, one HRT before TAH+BSO for palliative management and two HRT as a post surgical supplement.

DUB as you said, is only an 'Aartava vyaapat' and requires review / assessment of Dosha, Agni n dhaatu sampraapti vichaarana. This is the school of thought of Atreya (medical - palliative- HRT- dushtaartava chikitsa sootra). Assess DUB in terms of dosha and or other nidaanaarthakara systemic diseases.

is unclear whether Vds want to know what state of AARTAVA VYAAPAT or YONI VYAAPAT lead to Hysterectomy?

 OR

Other things are related to SCHOOL OF DHANVANTARI (surgical, TAH+BSO) For yoni vyaapats.

vijayalakshmi:

If DUB is there it may be adogha raktapitta. Or even in prameha or rakta meha condition artava atipravritti may be present, and simply treating atyarthava may not help. In such conditions you can try with dhanwantara 101 drops or dhanwantara gritha and vasavaleya or vasa Ghana. Sometimes even vasanta kusumakara Rasa can be added

Even application of chandana, or gopichandana or rakta chandana lepa may help a lot

If she is gaining wait means after surgery because of adyashana or samashana she might have medo dushti. You can advise panchamrith loha guggulu along with medo dushti chikitsa

yeligeti swetha:

Gd mng...vds.

in hysterectomy nd oophorectomy cases,pt may get srotho avarodha ..(rasavaha,arthavaha,manovaha)

arthovaha srotho avarodha: decreasing the hormones.

symptoms:hot flushes,lethargy,drowsyness.

rasavaha:due to hormonal imbalance affects lipid metabolism..leads wait gain..loss of appetite, indigestion, all rasavaha srotho janya diseases

manovaha:due to low level of oestrogen.may get irritation,sleeplessness.memory loss..like.

trt:rasavaha..deepana paachana.

arthovaha:virechana nd vasthi.

manovaha:dhara,pichu n nasya.

deepana paachana:regulates the liver function--balance the lipid metabolism.

virechana :..relieves the srotho obstruction.

vasthi:bruhmanam...like HRT.

dhara:for proper hormonal secretions from pituitary...

this is totally my opinion...nt by any reference.

any mistakes is there plz.plz..excuse.🙏.

डाॅ. उत्कर्ष आर्य:

Sir, what are the USG findings..

Dr.sunita:

Thank you madam ...

Here hysterectomy...oophorectomy ... and menopause/the condition or issues ...after cessation of cycles I asked  ... not Dub...

Dr.Vishwanath Dixit:

3. An experimental evaluation of antihyperlipidaemic activity of Kukkutanakhi (Aspidium

cicutarium sw-dryopteridaceae) - A folklore herb.

Scholar: Hardiek Mori Guide: Prof. K. Nishteswar

Madam it was from IPGT & RA, Gujarat Ayurved University, Jamnagar(M.D. (Ayu.) Projects in progress)

vijayalakshmi:

at the age of 28 she reached menopause?

Dr.Ganesh kumar:

I think for younger doctors / vaidya's this protocol of history taking / absence of history is necessery.

so as to get a feed back.

this is for everybody who wants clarification about tretment for various dieases

1. no. visits by the patient 1st, 2nd etc.

2. state age, matrial status, food veg/non veg

3. work

4. response of patient to earlier treatment / reports summery.

5. diet advised and why?

6. wheather you are pacticing ayurveda, allopathy or mixopathy.

if possible financial codition of patient. (to select sukumara / kasta jeevi)

I am not against mixopathy but it is needed in upashaya/ anupashaya; understanding chaya parkopa condtion of disease

finally your diagnosis and nidana Ayurvedic and / or allopathic. (you will know your decision is right or wrong)

this will help to understand the patient condition and to select line of approach.

Simply statement like DUB treatment is like we going to medical shop and asking for something for head ache.

It is not Ayurvedic approach at all.

if their is sombody from poona

there is one doctor (Dr. Ashok paranjape) who specilizes medical tretment in cases of surgical treament; like Arsha, vruddi, appendicitis, Udara etc,  Somebody can get his address or phone number; his advise will be valuable in cases where surgical intervention is advised.

Dr.sunita:

Surgery done last year only sir ... she was doing well ...HRT also given for some time ...and now from past 4_5 months she's complainning above ...gained 5_6  kgs  weight in  past 4 months ...

Dr.Ganesh kumar:

"one young lady aged 28 yrs ...moderately build ... in two years span ...uterus +one ovary and in next year the other ovary too removed bcos of cystic growths"

this is not history nor case history. Give information about dinacharya, Ahara kala, nidra

ahara matra etc.

Dr.venu:

Venugopal Rao:

Replace HRT with Phytoestrogens ......

Phytoestrogens are estrogens that occur in some plants.

Health benefits attributed to phytoestrogens include relief from menopausal symptoms and lowered risk of osteoporosis, heart disease and breast cancer. Phytoestrogens exert their beneficial effects through several mechanisms that slow cell growth and prevent inflammation. In addition, when acting on estrogen receptors, phytoestrogens behave differently from estrogen and more like Selective Estrogen Receptor Modulators (SERMs). An example of a well known SERM is the breast cancer treatment drug tamoxifen, which stimulates estrogen receptors in the uterus and bone, but suppresses them in the breast.

The most popular, best and effective plant that possess adequate Phytoestrogens is SHATAVARI.

In my practice I use Shatavari churna added with pippali churna.
